Mission Statement

 

It is the intent of this school to educate students to the extent required to pass the Ohio State Board of Cosmetology State Board Exams. The school also trains students to become quality cosmetologists or nail techs who enjoy their work and are able to make a long lasting and profitable career from the education they received.

 

The Owners

 

Owners: Scott Shaw and Bonnie Shaw have owned and operated a salon and spa in East Liverpool since 1988. Bonnie has been a licensed cosmetologist since 1979, and an instructor since 1990 she teaches theory classes and all aspects of salon and spa services, including nails, skin care, cosmetics, and hair. Mrs. Shaw continues to work at and run the salon with Scott Shaw.

 

The School

 

The school is located in Downtown East Liverpool Ohio, and was established in January 2003.Our goal is to train cosmetologists and nail techs to be ready to work in this exciting career. The owners also run a successful salon and spa in the Calcutta area and know what cosmetologists need to be successful.

We also are committed to making sure that each student is able to pass the Ohio State Board of Cosmetology exam. Cosmetology can be a rewarding and profitable career choice.

 

The school is in a 9000 square foot building but presently only occupies 6000 sq. ft. It is fully air conditioned and well heated. It is spacious and well equipped with the newest products and equipment. We have the newest education and learning techniques available, including an extensive library of DVD’s and tapes, and a CD Rom program of the material used. With permission, information can be accessed by Internet on one of the school computers. 3 class rooms, and a lounge for study and relaxing. A refrigerator and a microwave are available for those who wish to stay in for snack or lunch time.

Recruitment Policy

 

This school will not knowingly seek out, nor send literature to students for enrollment in our school, who are in another school of cosmetology that has similar courses of study.

 

 

 

Completion and Expectations

 

 

Physical Requirements

 

 The cosmetology program requires students to stand for much of the day, and to use repetitive hand movements.

The nail tech program requires extensive sitting. Some repetitious hand movements are also required in this course of study. Be aware of and adhere to all safety requirements at all times, when using chemicals.

 

 

Employment Assistance

 

Designer’s Beauty Academy does not guarantee job placement; however, our school will help students to obtain a job in their field of choice. We will maintain a list of salons in our area that are in need of employees, and notify, by phone or mail, our graduates of these openings. We have had a 100% job placement rate as of 2004 completion rates. We have many guest speakers in from area salons to give you ideas and offer you employment applications before you graduate. It is our goal to have every student employed before they take their State Board Exams

 

 

Employment Expectations

The Bureau of Labor states that jobs for stylists will be growing at an average pace through the year 2010. Jobs for nail technicians will be growing at a faster than average rate through the year 2010.

Designer’s Beauty Academy has a better than 80% licensure rate for those who take state board after graduation. We will remediate anyone for another 100 hrs for no charge should you fail state board and wish to retake it.

100% of our students have found job placement in the local area once they are licensed.  Beginning compensation for a stylist will be about  $7.00 per hour, plus tips, Beginning wages for a nail tech are not able to be calculated in hourly wages and depend on the hours worked and the salon in which you are employed. Evening hours for both professions pay a higher wages than day time, and Saturday hours are almost always required in order to obtain a job. There is a high rate of self employment for this profession and you will need to take the managing course if you wish to pursue this option for a cosmetologist, it is included in the nail tech course. Make sure you understand the employment situation in the salon you have chosen. Our staff will help you in this situation.

 

Completion rates for 2006

 

Designer's Beauty Academy graduates have a 75% passing rate on the state board exams for 2006. There is a 28% drop rate for students. 100% of all managing manicurist students who have taken boards have passed them. There is a 100% placement rate for all graduated students.

 

 

 

 

 

 

 

 

Admission Requirements

 

1.       Proof that the student is age 18 or over. ( except those enrolled under a training agreement)

2.     High school graduate, GED, or be able to pass a test on abilities to benefit (ATB)

3.     A driver’s license or other photo ID.

4.     Signed pre enrollment form and signed orientation forms

 

 

 

Eligible ATB applicants will be admitted after receiving a minimum score of 75% on the Wonderlic ATB test.

Registration requires a $50 fee that is non-refundable.

The kit cost for the cosmetology and nail programs are $400.00, the esthetician kit  is $500 and includes books, and other necessary items to work with.

You must have your kit with you in order to accrue hours every day. If you are a transfer student you must have the necessary items to work, a text book, and uniform.

Full payment is due at the beginning of the program. Students may make payments on the installment plan upon credit approval by the school; $500 down, and kit cost, registration fee plus monthly payments. The program must be paid in full when ½ of the program hours are finished.

 

Cosmetology training does not include acrylic, fiberglass, or gel, nail instruction nor does it include airbrushing classes, which are included in the managing manicurist program. If you want this training included you will have to purchase a nail tech kit at a cost of $100.

 

Students must attend class for a minimum of 3 hours each day that they attend, and a minimum of 20 hours a week, and need to be here during at least 1 class time each day. Full time is 30 hours a week.

The minimum time for full time students to complete the 1800 hour program is 60 weeks. The maximum time for full time for the 1800 hour program is 90 weeks.

The minimum time for part time in the 1800 hour program is 120 weeks and the maximum for part time is 178 weeks.

The minimum time for full time to complete the 1500 hour program is 50 weeks and the maximum time is 75 weeks. The minimum time for part time for the 1500 program is 100 weeks and the maximum is 149 weeks.

The minimum time for the 300 hour program is 10 weeks for full time and the maximum is 15 weeks for full time. The minimum for the 300 hour program for part time is 20 weeks and the maximum time for part time is 29 weeks.

 

The minimum time for the 750 hour program is 25 weeks for full time and the maximum is 38.5 weeks.

The minimum time for the 750 hour program is for part time is 50 weeks and the maximum is 79 weeks.

 

You must complete the program in the maximum time indicated. If you do not finish within the time allotted you will be charged an hourly rate of $6.00 per clock hour to complete the course chosen. This fee must be paid in advance for each month’s hours to be attended. Extenuating circumstances will warrant more time to complete the program without charge. This includes illnesses or any approved leave of absence, but they must be approved by the school administrator in advance.

 

Tuition is payable at the school office during business hours. All payments are credited to the students account and receipts showing balances are given. Being behind on payments will be a cause for termination

You may seek funding from a local bank. Monthly payments will be set up on an individual basis.

There is a $150 fee for anyone dropping a course or terminating. This fee applies also for anyone changing their course of study after beginning a course, or for the transfer of a student’s hours from this school to another.

 

 

 

 

Related Fields of Cosmetology

 

With a Managing Cosmetology or a Cosmetology license some of the other jobs available to you are Nail Tech, Esthetician, and Stylist, and Colorist, wig specialist, retail specialist, make-up artist, day spa worker, salon manger or owner, platform artist, relaxation massage expert, and with 1 year of experience a cosmetology instructor.

The managing manicurist license will prepare you for jobs in the salon industry as a manicurist, nail tech., pedicurist, you may specialize in nail art, airbrushing, or as manager or owner in a nail salon. You may also be prepared to sell many nail and spa related products. You may also be a platform artist for nails and with a years experience and a high school diploma you may apply for an instructor license, and in a school of cosmetology teaching a managing manicurist course.

A Managing Esthetician license will prepare you for the fast growing field of skin care you will be a make-up artist and be able to perform body treatments, skin treatments, and full body waxing and relaxation massage techniques including facials and full body relaxation massage. You will be able to work at photo shoots, in spas and salons, or have your own spa or skin care salon. You will also be qualified to work in department store make-up counters and sales and instruction of make-up for major cosmetic manufacturing companies and skin care consulting.

 

 

 

 

 

 

Credit for Previous Training

 

Credit for previous training is honored upon receipt of a certification from the Ohio State Board of Cosmetology. A letter showing your grades and performance reports from your previous school or schools will be needed also. The cost will be $6.00 a clock hour for the remainder of your hours, a $50 enrollment fee. If you are re-enrolling after leaving this school, all these rules and fees still apply. You must have a kit that is complete to our standards in order to attend class, a uniform and a current textbook.  Course requirements will be shortened accordingly and all records of previous education will become part of the student’s permanent record file. All other admissions requirements must be met. You need to take a 100 question test to show what areas you will need to study. 

 

 

 

 

 

 

 

 

Starting Dates and Orientation

 

You may enroll on any day of the week. You may start classes on the first Monday of every month for the 1800 hour and 1500 hour cosmetology courses. You may also start the managing manicurist program on the first Monday of each month.

You must attend the orientation program provided if you are going to start any program. It is held on the first Monday of the month at 9:00 and lasting approximately 2 hours, this is considered your first day of school and counts toward your hours, and you may attend for informational purposes before enrollment if you wish. This class is for all courses offered. A personal tour and information about the school may be done at any time by calling the school and making an appointment with the administrator, admissions director, or one of the instructors.

 

 

 

 

 

Licensing and graduation requirements

 

After completing the required satisfactory progress* in both attendance and academic study the school presents the students with a diploma in their field of study. To apply for state licensing the student must submit an application for examination. In addition to completing the hours for your course, the requirements for state licensing are as follows:

1.       10th grade education or equivalent and 16 years of age. (state board rules)

2.       Passing the state licensing exam with 75% or better

3.       1 2 ½”x 3 ½” photo

4.       the required fee $31.50(subject to change)

5.       work permit fee $7.50 ( subject to change)

6.       A $45 licensing fee if you pass the exam

7.       300 hours of nail related courses are required for the Managing Manicurist License.

8.       1500 hours of cosmetology related courses are required for a Cosmetology License.

9.       1800 hours of Managing cosmetology related courses are required for a Managing Cosmetologist License. The management hours are done in the last 300 hours of the program.

 

 

Designer’s Beauty Academy uses all of its resources to sufficiently prepare students for state licensing however, it does not guarantee licensing by the state upon graduation it is the responsibility of the student to pass the Ohio state board exam.

*Satisfactory progress: Students with a minimum of 75% average in theory work and Practical work is needed.

Our School is closed on the following days:

9. Other school closings will be announced at least 1 month in advance

School closures for extenuating circumstances. For cancellations due to weather listen to WTOV tv, WFMJ tv, and FROGGY 104 FM radio. For whether related closings, if the East Liverpool School District is cancelled we will be cancelled also. If there is a

 

 

 

 

 

School Hours

School will be open from 8:00am to 4:00 pm Mon. - Fri. & Sat. 9:00am-3:00pm. Evening and Saturday programs will be held only if a minimum number of 3 students are enrolled in night or Saturday classes. Please call if you are not attending by 8:00am days and 3:00 pm for evening students. We will close earlier if there are no students in attendance.

 

Textbooks and Equipment

 

 There will be no refunds or returns of the kit. Students will be responsible to purchase a new kit if it is lost or stolen or if anything in it needs to be replaced. Put name or initials on all property in your kit. A locker is provided at the school for valuables. Anyone caught stealing or in possession of someone else’s equipment or books will be expelled without a refund of tuition and the victim may file charges.

Any personal items left at the school for more than 30 days after graduation or discontinuation of classes will become the property of the school.

Destroyed, damaged, or borrowed school equipment must be paid for before graduating.

Textbooks that are property of the school may not be taken home, including hair styling books.

All textbook and workbooks are included in the price of the kit, which must be purchased before beginning classes. Lost or stolen textbooks must be replaced by the student.  

 

 

Evaluation and Advising

 

Students are advised regarding satisfactory progress and attendance on a regular basis, every 400 hours for the cosmetology program and every 100 hours for the managing manicurist program. Advising regarding licensing regulations, reciprocity, employment, and continuing education opportunities is available to students as it is needed. The administration office has a list of agencies for referral if a student needs other assistance. Regular progress evaluations include counseling regarding the student’s progress in attendance, theory, practical skills, communication skills and personal development. Student strengths and areas needing improvement are identified and plans for needed improvement are discussed.

 

 

 

Internship Program

 

For those students who are satisfactory or above in their attendance, appearance, attitude, achievement, and in full payment of all tuition, they will be offered the opportunity to be an intern in a local salon for 10% of their hours in the course of their choice. You may only intern upon completion of 50% of your hours. Cosmetology students may not perform chemical services until they have over 1000 hrs. The student has to be chosen by the salon and cannot accept payment for their services. Other circumstances may apply and will be discussed with the student when they reach that time in their course of study.

   

 

 

Refund Policy

 

1.       The school reserves the right to dismiss or suspend any student who fails to maintain continuous attendance, who is in default on tuition, who is not making satisfactory progress or whose conduct or activities violates the rules of the school.  

2.       If a student (or in the case of a student being under legal age, their parent or guardian) cancels their contract and demands their money back in writing within three business days of the signing of the enrollment agreement, all monies collected by the school, with the exception of the non-refundable $50 registration fee, shall be refunded. The postmark on written notification will determine the cancellation date or the date said information is delivered to the school’s administration in person. This policy applies regardless of whether or not the student has actually started training.

3.       If a student cancels their enrollment after 3 days of signing the enrollment agreement but prior to entering classes, they shall be entitled to a refund of all monies paid to the school less $50 the registration fee, within 30 days. Once a kit has been purchased there is no refund.

4.       Applicants rejected by the school are entitled to a full refund.

Terminated students will be charged a $150 administration fee for transfer or withdrawal. This includes changing courses after enrollment in one course and adding or changing to another.

 

6.       In case of school closing or cancellation of the course the student shall be entitled to a pro-rata refund of tuition. If a course is cancelled subsequent to a student’s enrollment, and before instruction in the course has begun, the school shall provide a full refund of all monies paid.

7.       If a student is absent for more than 30 days without an excuse that is accepted by the administration then a $25 re -enrollment fee will be charged. If a student does not notify the school that they have withdrawn, formal termination will occur 30 days from the last date of physical attendance.

8.       Students who do not return from a leave of absence, last date of leave is used as a termination date.

9.       If a student is absent for more than 30 days without an excuse that is accepted by the administration, the student will be withdrawn. The date of withdrawal will be 2 weeks after the last day attended without an excuse.

 

 

If student withdraws (Return of Title IV)

Students who receive Title IV aid (Federal Pell, Federal SEOG, Federal Subsidized and/or Federal Unsubsidized Stafford loans and PLUS) and withdraw from the institution are subject to the Return of Title IV aid regulations. (Federal law now requires that a student must "earn" the aid that they receive.) When a student withdraws from the institution, the institution is required to determine the amount "earned" using the Return of Title IV regulations. ( For Clock-hour schools this calculation is based on the number of hours the student attended divided by the number of hours for which the aid was disbursed.)

Up through the 60% point in each payment period or period of enrollment, a pro rata schedule is used to determine the amount of FSA funds the student has earned at the time of withdrawal. AFTER the 60% point in the payment period or period of enrollment, the student has earned 100% of the Title IV funds he or she was scheduled to receive during the period.  The "unearned" portion of the tuition will be returned to the appropriate Title IV program. The college will then bill the student for the amount of tuition that was returned in excess of the college's refund policy.

bulletThe "unearned" portion of the aid that was disbursed directly to the student will also be calculated. If applicable, the student will owe repayment to the Title IV programs. The student will receive an overpayment letter and will be given 45 days to make the repayment. The student will be ineligible for further Title IV aid until the funds are repaid or satisfactory payment arrangements have been made with the Department of Education.

Repayment of State Need Grant is also calculated for any student completing 60% or less of the hours in a given payment period.

For a student who withdraws after the 60% point-in-time, there are no unearned funds. However the institution will still complete a  Return calculation in order to determine whether the student is eligible for a post withdrawal disbursement.

The calculation of Title IV funds earned by the student has no relationship to the student’s incurred institutional charges.

 

Refunds

Refunds for financial aid recipients who do not receive Title IV aid, will be issued in accordance with the School’s Institutional refund policy as stated on the school’s enrollment/registration form.

Grievance Procedure

 

In accordance with the institution’s mission statement, Designer’s Beauty Academy will make every attempt to resolve any student complaint that is not frivolous or without merit. Complaint procedure will be included in the new student orientation and posted in the classroom; thereby assuring that all students know the steps to follow should the desire to register a complaint at any time. Evidence of final resolution of all complaints will be retained in school files in order to determine the frequency, nature, and patterns of complaints for the institution. The information will be used in evaluating institutional effectiveness and outcomes. The following procedure outlines the specific steps of the complaint process.

 

  1. The student should register the complaint in writing on the designated form provided by the institution within 60 days of the date that the act which is the subject of the grievance occurred.
  2. The complaint form will be given to the owners of the Academy.
  3. The complaint will be reviewed by corporate management and a response will be given in writing to the student within 15 days of receiving the complaint. The initial response may not provide for final resolution of the problem but will notify the student of continued investigation and /or actions being taken regarding the complaint.
  4. If the complaint is of such a nature that it cannot be resolved by the corporate management, it will be referred to
  5. Depending on the extent and nature of the complaint, interviews with appropriate staff and other students may be necessary to reach a final resolution of the complaint.
  6. In cases of extreme conflict, it may be necessary to conduct an informal hearing regarding the complaint. If necessary corporate management will appoint a hearing committee consisting of one member selected by the school who has had no involvement in the dispute and who may also be a corporate officer, another member who may not be related to the student filing the complaint or another student in the school, and another member who may not be employed by the school or related to the school owners. The hearing will occur within 90 days of committee appointment. The hearing will be informal with the student presenting his/her case followed by the school’s response. The hearing committee will prepare a report summarizing each witness= testimony had a recommended resolution for the dispute. Corporate Management shall consider the report and either accept, reject, or modify the recommendations of the committee.
  7. If the complainant is not satisfied with the committee’s decision, they may appeal to the Ohio State Board of Cosmetology.

 

 

 

 

 

Attendance Requirements and Tardy Policy

 

Students are required to be seated and ready to work when class begins. You must be in uniform and have your kit and your books or your hours will not be counted.

We will consider any extenuating circumstances on an individual basis.

You are required to call the school before 9:00am to report off.

Excessive tardiness and absentee days will be cause for termination from the school.  

 

If you do not complete the hours needed in the time allotted on your registration form, you will be charged $4.00 per credit hour to complete your course of study, payable before continuing the program.

 

You may not attend for less than 3 hours per day. Students will not be permitted to sign in if they cannot complete 3 hours. Allowances will be made at the discretion of the school for any extenuating circumstances.

 

The school does not deduct from clock hours for any reason and students receive credit for the hours they attend. However you may be put on probation for tardiness or absences.

 

You will need to keep a 67% average attendance calculated every 400 hours for the 1500 hr. and the 1800hr. programs and every 100 hrs for a 300 hr. program and every 200 hundred hours for the 750 hour program. See the satisfactory progress policy for more information. This is provided upon enrollment.

 

 

 

Probation or Termination

Probation is a warning that after the next offense you may be terminated. You maybe put on probation for any of the following reasons but not limited to these:

Poor Attendance

Poor Performance

Not adhering to rules and regulations listed in this handbook

Any violation of  rules or standards of conduct.

Termination by this school is unusual but will be done if a student is in violation of the above rules and is unwilling to abide by them after 2 attempts to remedy the violations.

 

Dress Code

 

All students must look neat and professional at all times. You will be working in close proximity to many other people therefore it is imperative that you obey all rules of personal hygiene and good grooming.

 

You must wear black pants and a white shirt. You must wear comfortable flat shoes. No open toed shoes are allowed.  Smocks should be worn to protect clothing while doing customers. White smocks for the estheticians, and black for the cosmetologists. None are required for nail services.

 

Hair must be cleaned and neatly styled to compliment your professional look. Students must have your hair already styled before class begins. Hair may be styled at school only as a practice service during clinic time. The school reserves the right to prohibit unprofessional piercings, tattoos or hair styles.

 

 

Rules and Regulations

1.       State Board does not allow the student to do anything during training hours that does not pertain to the activities that further the students’ education’s in the field of cosmetology. For that reason a student is not allowed to write personal letters, read non-related materials, or do anything that is not considered being part of the curriculum.

2.       You are not allowed to use the schools phone for personal use at all. There is a pay phone across the street and cell phones may be kept in your purse or kit and used during break times only. Inform your family and friends that you will only be given a message if it is an emergency and they are not to call you at the school.

3.       There is a no eating or drinking rule except for during breaks and in break rooms drinks are allowed in classroom if you clean up.

4.       There will be a zero tolerance for cursing and use of foul language, dirty talk or gestures. You will be sent home with the first offense and terminated with the second offense. It is very offensive to customers at any salon.

5.       Desk, dispensary, clean –up and decorating duties are a part of your training. On the days you are assigned you will be fully responsible. And you will not trade with another student.

6.       You must have all of your equipment at school whenever you are in attendance. You will not be allowed to borrow other students or the salon’s equipment. You will be held responsible for cleaning up after yourself. Any of your equipment left out will, if not stolen, will be handed in to the office.

7.       Students are not allowed to change the radio station or the temperature setting.

8.       Stealing will be cause for termination on the first offense. In addition the reason for your termination will be put on your permanent record forwarded to Ohio State Board of Cosmetology and given to any other school which you want to enter.

9.       The teachers, staff, and students of the school deserve your respect. Any verbal threats, intimidation, or loud and abusive language will not be tolerated and will be cause for immediate termination.

10.    No one may refuse to service a patron at any time for any reason.

11.    Anyone performing services not on the ticket will have to pay for it. Repeated offenses will be cause for dismissal.

12.    You must pay for any damaged or abused school property.

13.    This school reserves the right to prohibit any type of clothing, hair, face covering, jewelry, or any form of visible attire which we believe unprofessional or offensive. This will be at the sole discretion of the school’s faculty or staff.

14.    No one is allowed in the school area except students, instructors, patrons, and school employees. No friends, relatives, etc. may hang out around the school. They must wait outside or if it is not busy, in the reception area only.

15.    Students must be in attendance at least 3 hours before a lunch break will be given. The instructor will assign 30 min. lunch breaks. You must clock out for lunch breaks.

16.    Students not earning credit hours are not allowed at the school, and must leave the premises after clocking out.

17.    Drug or alcohol use is cause for termination. Any prescription drugs taken must be accompanied by a copy of the prescription which will be kept in your file.

18.    Any products used in the school will need to be approved by the instructor.

19.    At the discretion of the director, these rules may be changed and subsequently posted on the bulletin board.

20.    Ohio State Board rules and regulations are posted and need to be strictly adhered to by all students and employees. State Board regulations also state, “A student in a school of cosmetology who performs cosmetology services for the public outside such school is liable to prosecution under Section 4713-99 of the Revised Code.”

 

 

 

 

Records

Examinations are given on all subjects and marks are given to the students.  Grades, progress and attendance are recorded on grade and skill sheets. All records are kept in a locking file cabinet and are to be accessed only by authorized staff members. No information will be released unless permission is given by the student, parent, or their legal guardians if they are under 18yrs. old. The student can review these records by request from the office.  Any student must give a 24-hour notice for review of files. Request for copies of file contents must be in writing.

Requests for grade and hour transcripts must be signed by the student, parent or legal guardian, dated and specify the institution that the student wishes to receive the transcript.  There will be a $5.00 fee for requests by former students. We will not accept personal checks for these requests. In the event a balance is due to the school, no transcript will be provided. Authorization will be given to review student records by the accrediting agency, and government agencies as required by law.

There is a .25 fee for any and all copies made.

 

In order to provide employment we will give salons your name, address, phone number, dates of attendance, and any honors or awards you have earned at the school but not your actual grades. We will need to have the students consent or the consent of a minor’s parent or guardian, to do this. The school does not publish a student directory.

 

 

 

 

GRADING SYSTEM

 

 

There is a percentage semester grade average

 90_100%--A

 80-89%--B

 70-79%--C

 60-69%--D

 59% and below is an –F

 

Students will receive a satisfactory progress evaluation and grades every 400 hours for the cosmetology programs.

Every 200 hours for the 750 hour program and every 100 hours for the 300 hour program.

 

The grades are evaluated using a rubric made for the procedure being tested. Procedures are given a percentage grade also.

There is a written test on each chapter that is a multiple choice test.

Students must have a 75% to pass the chapter.   

Students who consistently receive below a 75% for 3 chapter tests wi1l have to be put on probation. If there is no improvement by the 6th test the student will have to be put in a leave of absence until you get remediation. Additional charges will be $10 an hour charge for remediation at the school; you may use an outside source.

All grades and hours will be kept on file for a minimum of 5 years

The objective of this course is to train students in manicuring and pedicuring of nails, sanitation, and sterilization, and the anatomy of skin, nails and muscles.
